(a) On or about October 20, 2022, at the jobsite located at 344 West South Street, Oconomowoc WI, an employee was tuck pointing on a residential building's chimney with a hand grinder without safety glasses, exposing the employee to a struck-by hazard.

Semper Fi Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1343380, citation number 1, item number 1 and was affirmed as a final order on October 3, 2018, with respect to a workplace located at S47 W25715 Wood Lilly Lane in Waukesha, Wisconsin 53189.

Semper Fi Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(2)(i), which was contained in OSHA inspection number 1572586, citation number 1, item number 1 and was affirmed as a final order on March 23, 2022, with respect to a workplace located at 4454 South 13th Street, Milwaukee Wisconsin 53221.  Semper Fi Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1343380, citation number 1, item number 2a and was affirmed as a final order on October 3, 2018, with respect to a workplace located at S47 W25715 Wood Lilly Lane in Waukesha, Wisconsin 53189.
