St. Mary's OH 45885. The employer Peterson Industrial Inc did not ensure employees were protected from falls while performing trenching activities atop a concrete manhole structure. Employees operating in this conditions were exposed to falls of 15 feet to the ground below.

29 CFR 1926.652(g)(1)(iii):Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: a) On or about October 19, 2022 at the address of 410 Defiance Rd. St. Mary's OH 45885. The employer Peterson Industrial Inc, failed to ensure employees were adequately protected from cave-ins while entering/exiting 16 feet deep trench. Employer installed 2 trench shield box approximately 10 feet apart exposing employees entering/exiting trench to make sanitary line (8" SDR 35) connections to cave-in hazards from unshielded soil.

St. Mary's OH 45885. The employer Peterson Industrial Inc, failed to ensure employees were adequately protected from cave-ins while making sanitary sewer line (8" SDR 35) connections in an approximately 16 feet deep trench. The employees were working at the bottom of a 16 feet deep trench inside of a trench shield box Model: TP5-8X16-K, Size: L16'x W4'x D8', Serial #: 16127 with 4 feet of unshielded soil from the bottom of the box to the trench base.
