29 CFR 1910.23(e)(1)(ii): The employer did not ensure that the steps and platforms of mobile ladder stands and platforms are slip resistant. Slip-resistant surfaces must be either an integral part of the design and construction of the mobile ladder stand and platform, or provided as a secondary process or operation, such as dimpling, knurling, shot blasting, coating, spraying, or applying durable slip-resistant tapes.  Establishment - The employer provided rolling platforms and step stools throughout the plant to access equipment.   These platforms and stools located in the waste water treatment room, by press E11B1, by press E11C1, and press in the adjacent to the machine in the back left line of 480 volt equipment, were missing rubber anti-slip feet and could move or slide when used by employees.

Establishment - The employer did not provide adequate Lock Out Tag Out training.   Current authorized employees had not been trained and did not follow the correct process when locking out the E11B1 Press.

Establishment - The employer allowed an authorized maintenance employee to work on the E11B1 press without adding his personal lock to the equipment, instead the press was locked out by his supervisor.

29 CFR  1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either.  Establishment - The employer did not ensure that secondary containers of hazardous chemicals had appropriate hazardous communication labeling.  The label on a secondary container of ink thinner on the EK4 machine had inadequate labeling, it was illegible and covered by spilled ink.

29 CFR  1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug found damaged or defective was used before being repaired.  Establishment - The cord on the EK4 machine was damaged and used prior to repair.  The cord was not equipped with a strain relief thus the extreme flexing/bending of the cord had caused the outer insulation to crack.
