29 CFR  1910.132(f)(1):The employer did not provide training to each employee who is required by this section to use personal protective equipment:  a. Or or about August 24, 2022 the employees required to wear PPE have not been trained on when PPE is necessary, what PPE is necessary; how to don, doff, adjust and wear PPE; the limitations of PPE;  and the proper care, maintenance, useful life and disposal of the PPE. Employees work with solvent 142, containing Hydrotreated Light Distillates. Employees wear safety glasses, gloves, ear plugs and goggles when performing different tasks.   b. 29 CFR  1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.   a. On or about August 12, 2022, the employer did not provide employees adequate heat resistant gloves when exposed to high heat and splash hazards from brass and lead.    b. On or about August 24, 2022 the employer did not select and require the use of appropriate chemical resistant gloves for employees whose hands are exposed to hazards from handling chemicals such as Solvent 142, containing Hydrotreated Light Distillates.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about October 15, 2022 the employer did not ensure that effective machine guarding was in place on the pneumatic press located in the piston assembly and brazing area. Employees are exposed to the pinch point between material and the ram/ die area. No guarding is present on the machine to protect employees from the hazardous area.  The Conn-Selmer, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(1) , which was contained in OSHA inspection number 1345036, citation number 2, item number 1 and was affirmed as a final order on February 27, 2019 with respect to a workplace located at 34199 Curtis Blvd., Eastlake, OH 44095.
