29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  On or about July 14, 2023, the employer did not ensure the documented and developed energy control procedure for the Dake Bow 2 Vertical Press Machine clearly and specifically outlined the specific procedural steps for shutting down the machine [per 1910.147(c)(4)(ii)(B)], specific procedural steps for the placement of lockout devices on the hydraulic energy source isolation device(s) [per 1910.147(c)(4)(ii)(C)], and specific requirements for testing the hydraulic energy source to determine and verify the effectiveness of the lockout out devices and other energy control measures [per 1910.147(c)(4)(ii)(D)].

29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about July 14, 2023, the employer did not conduct a periodic inspection of the developed and documented energy control procedure for the Dake Bow 2 Vertical Press Machine at least annually to ensure that the procedure and the requirements of the standard were being followed by employees engaged in service and maintenance activities, such as but not limited to, setting up the dies inside the press.

On or about July 14, 2023, the employer did not ensure sheet brass workers authorized to perform die set up activities on the Dake Bow 2 Vertical Press Machine, received training in the recognition of applicable hazardous energy sources and methods and means necessary for energy isolation and control of the press.

Conn-Selmer, Inc., was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.147(d), which was contained in OSHA inspection 1345036, Citation 1, Item 2 and was affirmed as a final order on February 27, 2019, with respect to a workplace located at 34199 Curtis Blvd, Eastlake, Ohio.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about July 14, 2023, the employer did not ensure employees performing service and maintenance activities on the Dake Bow 2 Vertical Press Machine applied energy controls by; turning off or shutting down the machine [per 1910.147(d)(2)], isolating the electrical and hydraulic energy sources [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], releasing the stored hydraulic energy [(d)(5)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to setting up dies in the press.    Conn-Selmer, Inc., was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d), which was contained in OSHA inspection 1345036, Citation 1, Item 2 and was affirmed as a final order on February 27, 2019, with respect to a workplace located at 34199 Curtis Blvd, Eastlake, Ohio.

29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about July 14, 2023, the employer did not provide point of operation guarding that prevented the operator of the Dake Bow 2 Vertical Press Machine from having any part of his body in the danger zone during the operating cycle.  Conn-Selmer, Inc., was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.212(a)(1), which was contained in OSHA inspection number 1629671, citation number 2, item number 1 and was affirmed as a final order on February 28, 2023, with respect to a workplace located at 34199 Curtis Blvd, Eastlake, Ohio.
