The Roof - On or about October 28, 2022, employees were accessing the roof by means of extension ladders for the purpose of installing shingles on a multilevel single-family house without the use of fall protection. The ladders were not extended 3 feet above the landing and employees were exposed to fall hazards in excess of 10 feet.  No abatement certification required.
