(b) On or about November 1, 2022, point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle while operating the Precision Mathews Drill Press-Model: PM-935TV located in the Heater Module Maintenance Area. (c) On or about November 2, 2022, point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle while operating the MasterForce Sander-Model: 240-0054 located in the Machine Shop Area. (d) On or about October 24, 2022, point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle while operating the Jet Milling/Drilling Machine-Model: JMD-18 located in the Machine Shop Area. (e) On or about October 3, 2022, point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle while operating the Craftsmen Drill Press-Model: 34984 located in the Machine Shop Area.
