(a)at the Vaccu-Transfer area of the warehouse located at 5829 Pepsi Place, Jacksonville Florida, 32216: The employer exposed employees to crushing hazards in that a powered industrial truck was left unattended with the load engaging means not lowered. The controls were neutralized and the power was shut off, the brakes were set and the key left in the ignition. The employee was working approximately 120 feet away from the point of operation.
