29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) Allegheny Wood Works, Inc. - On or about 11/10/22, and at times prior thereto, a specific lockout/tagout procedure was not in place for the chop saw. b.) Allegheny Wood Works, Inc. - On or about 11/10/22, and at times prior thereto, the company's general lockout/tagout procedures were not listed to the appropriate step by step order of lockout operations. The stored energy energy sources and the validation of energy isolation (i.e. testing the machine) were reversed. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
