Employees were engaged in activities related to framing at a new construction residential building located at 6310 N. 170th Ave., Omaha, NE 68118, was exposed to struck-by and fall hazards in that the employees were not instructed in the recognition of the hazards associated with operating a powered industrial truck near employees or with a powered industrial truck mounted scaffold, and the methods to mitigate those hazards.
