29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. (a) Production area: On or about 11/23/2022, wood debris was allowed to accumulate on the Production Area floor. An employee operating a Raymond stand-up counter-balanced forklift struck a piece of this wood debris, causing the forklift to jump/pitch, resulting in the operator's foot slipping off of the standing platform and becoming trapped under the back edge of the forklift.
