29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  a) On or about November 22, 2022, the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening #1 along east side wall of 4th deck access.   b) On or about November 22, 2022, the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening #2 next to single rebar at middle of 4th deck.   c) On or about November 22, 2022, the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening #3 at a group of rebar at middle of 4th deck.  d) On or about November 22, 2022, the employer did not protect employees from stepping into unprotected floor holes in the building, floor opening #4 at rebar by south wall.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
