29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  a) On February 4, 2025, employees were exposed to fall hazards of approximately 4 stories while working in a hoist area.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records  United Builders Group Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1) which was contained in OSHA inspection number 1770832, citation 1, item 1, and was affirmed final order on January 23, 2025, with respect to a workplace located at Belmont & 80th Ave, River Grove, IL 60171.  United Builders Group Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CRF 1926.501(b)(13) which was contained in OSHA inspection number 1507562, citation 1, item 2, and was affirmed final order on October 20, 2021, with respect to a workplace located at 4214 W. Belmont Ave, Chicago, IL 60641.
