29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a)  On or about December 1, 2022, employees were exposed to fall hazards of approximately 17 feet on the backside of the residential property and 10 feet on the front side of the residential property.  The employer did not ensure the personal fall arrest system was inspected prior to use.  Anchor points on the roof's peak were not installed properly, in-that the anchor point was only nailed in place on one side of the roof anchor bracket, and the lanyards in use were damaged and deteriorating.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about December 1, 2022, employees conducting roofing work on an existing residential roof with a 6:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10 feet on the front side of the house and 17 feet on the back side of the house.  The HD Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1598323, citation number 2, item number 1 and was affirmed as a final order on August 2, 2022, with respect to a workplace located at 2844 Fox Lake Circle, Unit 101, Waukesha, WI 53189.  The HD Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1568611, citation number 1, item number 1 and was affirmed as a final order on April 21, 2022, with respect to a workplace located at 1767 Derby Lane in Oconomowoc, WI 53066.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about December 1, 2022, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work. The HD Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1568611, citation number 1, item number 2 and was affirmed as a final order on April 21, 2022, with respect to a workplace located at 1767 Derby Lane in Oconomowoc, WI 53066.
