1926.454(c)(3): The employer did not to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) 1201 Walnut Bottom Road, Carlisle, PA - End of building - Employee installed vinyl siding from a pump jack scaffolding system 18 feet above grade without fall protection on the ends and back jack, on or about October 4, 2023. (b) 1201 Walnut Bottom Road, Carlisle, PA - Front of building - Employee installed blue vinyl siding from a pump jack scaffolding system 18 feet above grade without fall protection on the ends and back, on or about October 4, 2023. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.451(e)(8):Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: (a) 1201 Walnut Bottom Road, Carlisle, PA - Jobsite - At the end of the day an employee lowered one end of the pump jack scaffolding system, to a second story window to egress platform through a window, where the direct access was approximately 19 inches, on or about October 4, 2023. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
