(a) On December 7, 2022, at 237 12th Street NW in West Fargo, North Dakota. Employees operating  an overhead crane, using a Safe Way synthetic web sling rated for a 5,000 pound choker hitch. The sling was loaded beyond its rated capacity resulting in sling failure.
