29 CFR  1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a.) Hot End, Line B8,  - On or about December 9, 2022, the employer failed to ensure that an energy control procedure was developed and documented for the control of hazardous energy when employees are engaged in service and/or maintenance tasks such as changing a plunger cylinder on the Emhart Glass IS Bottle Machine, Serial No. 14 11 035 02.  Employee were thereby exposed hazards associated with rotating and moving parts.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a.) Cold End, Line B8 - On or about August 2, 2022, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.  A mechanic sustained an electric shock while removing electrical feeder wires from the South API Safepacker machine.    b.) Hot End, Line B8 - On or about December 9, 2022, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.  A machine repair apprentice changed a plunger cylinder on the Emhart IS Glass Machine, Serial No. 14 11 035 02.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.)  Cold End, Line 8 - On or about August 2, 2022, employees were exposed to electric shock hazards while dismantling the API Safepacker machine.   The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a.) Cold End - On or about August 2, 2022, and dates prior to, authorized employees performed service and/or maintenance on the Line B8 API Safepacker machine and other equipment.  The employer did not ensure that periodic inspections of the energy control procedures were performed at least annually as required by the standard.
