29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) Hot End, Line B8 - On or about January 31, 2024, while performing service and maintenance on the Line B8 Emhart Glass IS Bottle Machine, an employee was exposed to machine hazards associated with moving parts, including pinch-point, crush, struck-by, and amputation, while performing a mold change on the Line B8 Emhart IS Glass Machine. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

A machine operator changed the mold without properly shutting down the Emhart Glass IS bottle machine. b.) Hot End, Line B7 - On or about January 31, 2024, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Hot End, Line B8 - On or about January 31, 2024, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. A machine operator cleared a jam on the Emhart Glass IS bottle machine. Gerresheimer Glass, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1638360, citation number 1, item number 2, and was affirmed as a final order on February 17, 2023, with respect to a workplace located at 1131 Arnold Street, Chicago Heights, IL 60411.
