Building 601, Front Desk Custodian Closet: On or before December 14, 2022, an employee handled hazardous chemicals such as Peroxide Multi Surface Cleaner and Disinfectant while conducting custodial tasks. The employer did not ensure the wall mounted eye wash station provided 15-minute continuous flow for drenching for emergency aid and it was blocked from direct access by a mop bucket and a mop. Employee was exposed to eye hazard(s).

In the following locations:  a) Front Desk Custodian Closet: Safety glasses with side shields, goggles, and/or face shield  were not utilized while conducting wiping and cleaning tasks.   b) Second floor chemical refilling station: safety glasses with side shields, goggles and/or face shields were not utilized while replacing 0.34 gallons chemical cartridges, and then refilling 32 oz. spray bottles.  Employee was exposed to serious eye hazard(s).
