On the roof of a temporary residential worksite, at 3117 Sedona Trail, Jacksonville, Florida 32208: On or about December 12, 2022, the employer exposed eight employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using pneumatic nail guns to install roofing shingles.

On a 6:12 pitched roof of a temporary residential worksite, at 3117 Sedona Trail, in Jacksonville, Florida 32208: On or about December 12, 2022, the employer exposed eight employees to fall hazards ranging from 9 to 18 feet, in that fall protection was not utilized by employees while installing roof shingles.
