On a 4:12 pitched roof of a temporary residential worksite, at 350 W 65th Street, Jacksonville, Florida 32208: On or about March 14, 2023, the employer exposed employees to fall hazards ranging from 8 to 15 feet 6-inches, in that fall protection was not utilized by employees while performing roofing activities.
