29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a.  On a 4:12 pitched roof of a temporary residential worksite, at 350 W 65th Street, Jacksonville, Florida 32208: On or about March 14, 2023, the employer exposed employees to fall hazards ranging from 8 to 15 feet 6-inches, in that fall protection was not utilized by employees while performing roofing activities.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. Left side of home near door, located at 350 W 65th Street, Jacksonville, FL 32208: On or about March 14, 2023, the employer exposed employees to fall hazards, in that, the 16-foot extension ladder was only raised one rung above the landing.
