29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:  a. On the roof of a temporary residential worksite, at 9513 Arbor Oak Lane, Lot 11, Jacksonville, Florida 32208: On or about February 27, 2023, the employer exposed three employees to head injuries hazards caused by flying objects, in that hard hats were not utilized by the employees, while working underneath other employees picking up debris.

On the roof of a temporary residential worksite, at 9513 Arbor Oak Lane, Jacksonville, Florida 32208: On or about February 27, 2023, the employer exposed five employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using pneumatic nail guns to install roofing shingles.  b. On the roof of a temporary residential worksite, at 9513 Arbor Oak Lane, Jacksonville, Florida 32208: On or about February 27, 2023, the employer exposed one employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee using the DeWalt saw to cut plywood.

On a 6:12 pitched roof of a temporary residential worksite, at 9513 Arbor Oak Lane, in Jacksonville, Florida 32208: On or about February 27, 2023, the employer exposed five employees to fall hazards ranging from 9 to 18 feet, in that fall protection was not utilized by employees while installing roof shingles.

29 CFR  1904.41(a)(1): the employer had not electronically submit information from their recordkeeping forms required under this part once a year and no later than the date listed in paragraph (c) of this section, March 2, 2023:  a. Redtail Roofing, LLC, located at 7643 Gate Parkway, Suite 104-177, Jacksonville, Florida 32256: On or about March 10, 2023, and times prior thereto, the employer failed for calendar year 2022, to electronically submit information from the OSHA Form 300A or equivalent by March 3, 2023.  The establishment employed 53 employees and was classified in the North American Industrial Classification System as 238160 during calendar year 2022.
