29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a- 2 Sage Way, Mount Laurel NJ: On or about 01/05/23 two workers were exposed to fall hazards, up to and  in excess of  16 feet to the ground surface below, when working/walking from a job made wooden box that was set on the forks, attached to the back rest with two nylon straps, of an all terrain fork truck's elevated forks. This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform of the size, weight, specifications, etc. in use by the employer on the day of the inspection.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a- 2 Sage Way, Mount Laurel NJ: On or about 01/05/23 two workers were exposed to fall hazards, up to and  in excess of  16 feet to the ground surface below, when working/walking from a job made wooden box that was set on the forks, attached to the back rest with two nylon straps, of an all terrain fork truck's elevated forks. While working from this box, the workers were exposed to falls as a result of the box not having guardrails running the full perimeter of the box.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
