29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about January 10, 2023, employee(s) conducting rough framing work while standing on the 2nd floor of structure, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12 feet, seven (7) inches.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: (a) On or about January 10, 2023, employee(s) working outside a framed residential structure did not insure an eight (8) foot self supporting portable step ladder was used as designed. The eight (8) foot step ladder was left in the closed position and leaned against a wall next to a framed window hole, while the employee used the ladder to reach above. This exposed employee to a tip and fall hazard.
