29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about April 24, 2025, at a new construction residential jobsite located at 1211 W Kiddle Lane, Union Grove, WI 53182, the extension ladder being used by employees to access the roof of the structure at or near the front door did not extend at least three feet above the upper point of contact with the eave nor was it secured in place. This exposed employees accessing and egressing the roof, to a fall hazard of approximately 11 feet 2 inches onto gravel and building materials.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about April 24, 2025, at a new construction residential jobsite located at 1211 W Kiddle Lane, Union Grove, WI 53182, an employee conducting roof sheathing/decking work on an 8:12 pitch roof was not protected from falling to the lower level by guardrail systems, safety net system, or personal fall arrest system. This exposed the employee to a fall hazard of approximately 11 feet, 2 inches onto dirt, gravel, and building materials. Platinum Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1733020, citation number 1, item number 1, and was affirmed as a final order on August 19, 2024, with respect to a workplace located at 502 Kiddle Lane, Union Grove, WI 53182. Platinum Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1642832, citation number 1, item number 1, and was affirmed as a final order on February 14, 2023, with respect to a workplace located at 1081 58th Road, Union Grove, WI 53182.
