29 CFR 1910.147(f)(1):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed: Location: Production floor, 9 Creek Brook Drive, Haverhill, MA. a) On or about 1/5/23, an employee conducting testing on the Shanklin F5 machine was not protected by barrier guards or other measures from an open chain and sprocket, which the employee contacted and received a severe hand injury. Or in the alternative; 29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): a) On or about 1/5/23, an employee conducting testing on the Shanklin F5 machine was not protected by barrier guards or other measures from an open chain and sprocket, which the employee contacted and received a severe hand injury.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Location: Production floor, 9 Creek Brook Drive, Haverhill, MA. a) On or about 1/17/23, employees were exposed to the sealing bar mechanism, which includes a heated blade. Employees attempting to clear jams or misaligned containers could be contacted and injured by the bar.
