29 CFR 1910.145(c)(3):Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): Location: Retort Room On or about 12/5/24, the employer did not provide safety instruction signs for the following instances: a) An employee placing their hand on the door of the retort machine was not instructed by signage to keep hands clear of the point where the door mechanism locks to the vessel itself. The employee received a fingertip amputation injury. b) Employees entering the retort pit are potentially exposed to hot water that is discharged from the retort vessels after the cooking process. Employees are not instructed by signage of the hazard.

29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination: Location: Retort Room On or about 12/5/24, the employer did not document that all hazards in the permit space had been eliminated through a written certification prior to entry. Employees loading the retort vessel with potatoes physically enter the vessel to push carts of potatoes down the vessel, entering the permit required confined space.
