Operators were performing set up activities and removing fixed guards during set up with no energy isolating procedures.

29 CFR 1910.147(d): 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): On or about January 12, 2023, the employer did not ensure the Operator A, who was performing adjustments, implemented energy control procedures by; preparing the machine for shutdown [per 1910.147(d)(1) requirements], turning off the machine [(d)(2)], isolating the machine from the energy source [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to cleaning the machine [(d)(6)].

29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: On or about January 12, 2023, the employer failed to provide training to ensure affected and authorized employees understood the purpose and function of the energy control program and employees had acquired the knowledge and skills required for the safe application, usage, and removal of energy controls. The employer engages in the service and maintenance activities on machinery and equipment including, but not limited to; bag machines with electrical and air energy sources. Employees are required to remove a fixed barrier guard to perform adjustments/set up activities.
