29 CFR  1926.102(a)(1):  Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At a worksite located at 3109 SW Merriam Dr, Lees Summit, MO, 64081, employees were exposed to eye injury hazards.  The employer exposed the employees to these hazards by allowing them to use pneumatic nail guns without wearing eye protection.  Leiva Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1588810, citation number 1, item number 2 and was affirmed as a final order on 06/07/2022, with respect to a workplace located at 25178 W. 91st Terr.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a worksite located at 3109 SW Merriam Drive, Lees Summit, Missouri, 64087, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to work at heights greater than six feet without the use of appropriate fall protection.     Leiva Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1588810, citation number 1, item number 3 and was affirmed as a final order on 06/07/2022, with respect to a workplace located at 25178 W. 91st Terr., Lenexa, KS.

29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  At a worksite located at 1209 N Paragon Rd, Springfield MO an employer exposed employees to hazardous conditions.  The employer exposed the employees to these conditions by not ensuring that employees were trained in the recognition of hazards that may occur due to a lack of personal protective equipment.
