29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  Bowling Pinsetter (20), Brunswick GSX: On and/or before January 18th, 2023, the employer did not conduct a periodic inspection of it's lockout tagout program as required by this standard. This condition potentially exposed employee(s) to crush and/or amputation hazards.

29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.   Bowling Pinsetter, Brunswick GSX: On and/or before January 18th, 2023, the employer did not ensure that locks and tags were placed on each energy isolating source on multiple pinsetter machines. Multiple machines were down for repair and none has locks or tags on them. This condition potentially exposed employees to amputation or crush hazards when performing maintenance.   The condition applied to the following locations: a) Machine 20 b) Between Machine 18 & 19 c) Machine 11 d) Machine 9  e) Machine 4 f) Machine 1

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   Fort Irwin Bldg. 905: Bowling pinsetter machines pulley with belt systems were less than seven feet from the working surface and were missing their guard(s). This condition potentially exposed employee(s) to nip point hazards in the following locations:  a) Machine 20 b) Between Machine 18 & 19 c) Machine 11 d) Machine 9  e) Machine 4 f) Machine 1  The Army Morale, Welfare and Recreation Program was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.219(d)(1), which was contained in OSHA inspection number 1390508, citation number 1, item number 1-a, and was affirmed as a final order on 05-21-2019, with respect to a workplace located at Bldg. 905, Desert Winds Bowling Center, Fort Irwin, CA 92310.

29 CFR  1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling.  Mechanic Office: On and/or before, January 18th, 2023, the employer did not ensure that a personal space heater was plugged in properly into the wall as mentioned on it's label. It was plugged into a relocatable power tap against its listed and labeling requirements. This condition potentially exposed employee(s) to fire hazards.

29 CFR  1910.305(a)(2)(i)(B): Temporary wiring was used for a period exceeding 90 days.   Back of bowling alley, above pinsetter machines:  On and/or before January 18th, 2023, temporary lighting installations of 600 volts, nominal, or less was not removed after exceeding the 90 day period allowed by the standard. The yellow string lights were plugged in for more than 3 years. This condition potentially exposed employee(s) to fire hazards.

29 CFR  1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:  Chemical Storage Room: On and/or before January 18th, 2023, the employer did not ensure that an electric cable was removed from walking working surface. The electric cable was in the path where the mechanic would walk to grab chemicals while still plugged in. This condition potentially exposed employee(s) to trip and fall hazards.
