29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d) or replaced:  (a) Harvard Integrations, LP at 27157 470th Avenue, Tea SD, 57064: On or about and at times prior to January 20, 2023, the employer did not remove a 8-foot ladder from the North Building production floor. This condition exposes employees to using ladders that were broken, which could cause a fall hazards.  (b) Harvard Integrations, LP at 27157 470th Avenue, Tea SD, 57064: On or about and at times prior to January 20, 2023, the employer did not remove a ladder from the North Building production floor. This condition exposed employees to using ladders that were broken, which could cause a fall hazards.  (c) Harvard Integrations, LP at 27157 470th Avenue, Tea SD, 57064: On or about and at times prior to January 20, 2023, the employer did not remove a yellow Rock River ladder from the South Building production floor. The ladder had damage to the rear support bar.

29 CFR 1910.134(g)(1)(i)(A): The employer did not ensure that employees with facial hair between the sealing surface of the facepiece and the face did not wear respirators with tight-fitting facepieces:   (a) Harvard Integrations, LP at 27157 470th Avenue, Tea SD, 57064: On or about and at times prior to January 20, 2023, the employer did not ensure employees conducting spraying operations with respiratory protection did not have hair between their face and the sealing area of the respirator.  This exposed employees to respiratory hazards.

29 CFR 1910.252(b)(2)(iii): The employer did not ensure that proper protection from arc welding rays:  (a) Harvard Integrations, LP at 27157 470th Avenue, Tea SD, 57064: On or about and at times prior to January 20, 2023, the employer did not replace a damaged welding curtain in the South Building in close proximity to the paint booth. This condition exposes welders or other employees walking by the damaged welding curtain to possible eye damage.
