In the rear of the structure located at 9929 Intervention Lane, Lot 001, Jacksonville, FL: On or about January 26, 2023, the employer exposed employees to a fall hazard, in that a 16-foot Werner extension employees used to gain access to the top of a structure did not extend 3 feet above the eave, exposing employees to a fall hazard.
