On top of a structure at 9929 Intervention Lane, Lot 001, Jacksonville, FL: On or about January 26, 2023, the employer exposed three employees to fall hazards, in that fall protection was not utilized by employees performing framing operations.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. In the rear of the structure located at 9929 Intervention Lane, Lot 001, Jacksonville, FL: On or about January 26, 2023, the employer exposed employees to a fall hazard, in that a 16-foot Werner extension employees used to gain access to the top of a structure did not extend 3 feet above the eave, exposing employees to a fall hazard.
