On the roof of a temporary residential worksite, at 107 Canopy Forest Drive - Lot 69, St. Augustine, Florida 32086, On or about March 21, 2023, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to sheath the roof. b. On the roof of a temporary residential worksite, at 107 Canopy Forest Drive, - Lot 69, St. Augustine, Florida 32086, On or about March 21, 2023, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees while using saws to cut sheets of plywood and 2 x 4's.

On front porch, at a temporary residential worksite located at 107 Canopy Forest Drive, Lot 69, St. Augustine, Florida 32806: On or about March 21, 2023, the employer exposed an employees to fall hazards, in that, the employees used the top step of a Werner 6-foot A-frame ladder to reach the rafters, exposing himself to fall hazards. b. Inside the front room, at a temporary residential worksite located at 107 Canopy Forest Drive, Lot 69, St. Augustine, Florida 32086: On or about March 21, 2023, the employer exposed an employee to fall hazards, in that, employees used the top step of a Werner 8-foot-a-Frame ladder to reach the rafters, exposing himself to fall hazards.
