(b) Lauderdale Hamilton Super Chop Saw No. 1 - On or about January 26, 2023 the employer did not ensure that a chop saw was properly guarded, exposing employees to a struck-by hazard. (c) Cemco Drill - On or about January 26, 2023 the employer did not ensure that the drill press was properly guarded, exposing employees to a struck-by hazard.

(a) Facility - On or about January 26, 2023 the employer had not developed and implemented a written hazard communication program governing the usage of hazardous chemicals, such as but not limited to, Chevron Delo Oil, exposing employees to eye and skin irritation when using these products.
