29 CFR 1910.26(d): Measures, such as wheel chocks or sand shoes, are used to prevent the transport vehicle (e.g. a truck, semi-trailer, trailer, or rail car) on which a dockboard is placed, from moving while employees are on the dockboard. At the establishment located at 2980 South Oakwood Road, Oshkosh, Wisconsin; the employer did not ensure that measures, such as dock locks or wheel chocks, were used to prevent a semi-trailer on which a dock leveler was placed from moving during loading or unloading operations, exposing employees to caught between or under and fall hazards in the following instances: a. On or about and at times prior to January 27, 2023, at the Shipping and Receiving docks, employees entered semi-trailers while operating forklifts without a means to prevent the semi-trailer from moving. b. On or about January 27, 2023, at Dock 7, an employee entered a semi-trailer that was connected to a truck while operating a CAT forklift (Model: C5000-LP, SN: AT9020253) without a means to prevent the semi-trailer from moving. The truck that was connected to the semi-trailer was idling with the driver seated behind the wheel.
