29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use. Location: Unit 7 Lilac Ave. Bellingham Ma. On January 25th 2023 an employee was exposed to a fall of more than 11 feet from the top of the ladder, to the asphalt driveway below, where the ladder was not inspected by a competent person.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: Unit 15 Lilac Ave. Bellingham Ma. On or about January 27th 2023 employees were exposed to fall hazards of more than six feet (6) while performing residential framing and trim work, without the use of a personal fall arrest system. Instance 1: An employee fell approximately eleven feet ten inches on January 25 2023, while accessing the roof from a ladder, without the use of a personal fall arrest system. Instance 2: January 27th employees were exposed to fall hazards of greater than 19 feet while performing framing work on the second floor without the use of personal fall arrest systems.
