29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Location: 155 Palmer Road, Plympton MA. On or about April 25, 2025 employees were exposed to falls from heights up to 20 feet to the ground while accessing an upper landing surface where an extension ladder was used to access the roof where the ladder side rails did not extend at least 3 feet above the edge of the roof.

29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Location : 155 Palmer Road Plympton MA On or about April 25, 2025, employees were exposed to fall hazards up to an estimated 20 feet while working on a commercial roof where fall protection measures were not in place, by any means. The TN Carpentry INC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1751258, citation number 2, item number 1 and was affirmed as a final order on (date), with respect to a workplace located at 212 Nantasket Road, Hull, MA. 02045.
