29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   a- Building 1000, Mount Laurel NJ: A Worker was exposed to fall hazards, up to 8 feet to the floor surface below, when using an A-framed ladder in the closed position.  The worker was using this ladder  to access and egress an interior scaffold and exterior wall top plates.  Condition observed on or about 01/31/23.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a- Building 1000, Mount Laurel NJ:  A worker was not protected from falling when walking and working on a commercial structure's outer wall top plates without the use of fall protection.  As such, this worker was exposed to fall hazards of approximately 27 feet to the ground surface below. Condition observed on or about 01/31/23.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:   a- Building 1000, Mount Laurel NJ:   A worker was not protected from falling when walking and working on the top of pitched roof trusses while wearing a full body harness with a lanyard attached to it and the other end connected back to itself, with the hook, as it was wrapped around the connector plate & chord within a wooden roof truss. As such, this worker was exposed to fall hazards in excess of 10 feet to the flooring below. Condition observed on or about 01/31/23.  b - Building 1000, Mount Laurel NJ:  A worker was was not protected from falling when walking and working within the inner chords of roof trusses while wearing a full body harness with a lanyard attached to it and the other end connected back to itself, with the hook, as it was wrapped around the connector plate & chord within a roof truss. As such, this worker was exposed to fall hazards in excess of 10 feet to the flooring below. Condition observed and noted on or about 01/31/23.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
