29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Jobsite located at 2002 Brown St, Philadelphia, PA - On or about January 31, 2023, the employer did not provide a stairway or ladder at all personnel points of access where there is a break in elevation of more than 19 inches. The employees were required to utilize a wooden plank fastened against the wall diagonally as a means to access and egress the upper level approximately 48 inches from the ground, thereby exposing employees to fall hazards.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite located at 2002 Brown St, Philadelphia, PA - On or about January 31, 2023, the employer did not ensure that employees were protected from fall hazards by the use of a guardrail system, safety net system, or personal fall arrest system. Employees were conducting framing operations from the 2nd level of a residential construction building without the use of fall protection, exposing employees to falls of approximately 15 feet above the ground level. Solutions Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1553242, citation number 1, item number 1-a, and was affirmed as a final order on 3/8/2022, with respect to a workplace located at 2500 E Tioga St Philadelphia, PA 19134.
