29 CFR  1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   (a)D & S Welding, LLC at  48277 249th Street, Sherman, SD 57030:  On or about and at times prior to February 1, 2023, the employer did not ensure that electrical equipment was free from damage in that the electrical cord on the quick disconnect of the Piranha Ironworker was separating from the plug and exposed the inner connectors.  This condition exposed employees to shock hazards.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  (a)D & S Welding, LLC at 48277 249th Street, Sherman, SD 57030:  On or about and at times prior to February 1, 2023, the employer did not ensure the Argon/Carbon Dioxide welding tanks were protected from damage or being knocked over.  Tanks were being stored chained together but were not secured to a wall or stored in an enclosure.  This condition exposed employees to potential struck-by hazards associated with the unsecure pressurized tanks.  D & S Welding, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.253(b)(2)(ii), which was contained in OSHA inspection number 1609792, citation number 1, item number 5 and was affirmed as a final order on November 18, 2022, with respect to a workplace located at 48277 249th Street, Sherman, SD 57030.

29 CFR  1910.134(h)(2)(i): All respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they were not packed or stored to prevent deformation of the facepiece and exhalation valve:  (a)D & S Welding, LLC at 48277 249th Street, Sherman, SD 57030:  On or about and at times prior to February 1, 2023, the employer did not ensure that respirators were stored and maintained properly or in accordance with the manufacturer's recommendations.  This condition exposed employees to health hazards associated with improper storage, loss of fit due to respirator deformation, and contamination.

This condition exposed employees to hazards associated with ingestion, inhalation or absorption of hazardous chemicals.
