29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  At the residential construction site, employees performing framing activities using a metal ladder with side rails that did not extend at least 3 feet above the edge of the roof were potentially exposed to fall hazard greater than 11 feet.

At the jobsite, employees were not wearing hardhats while working below other employees that were installing framing material to a residential house, exposing employees to struck-by hazards from falling objects. At the construction  site, employees were using a DeWalt skill saw that had the outer sheathing pulled away exposing internal wiring. Employees were exposed to an electrical shock hazard.
