29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) At Flooring Production: On or about April 28, 2023 and at times prior, the employer exposed employees to amputation and caught-in hazards, in that guarding was not provided for chains and sprockets on the Black Brothers Glue Spreader while employees operate the machine. b) At Flooring Production: On or about April 28, 2023 and at times prior, the employer exposed employees to amputation and caught-in hazards, in that guarding was not provided for chains and sprockets on the End Matcher while employees operate the machine.

29 CFR 1910.1200(e)(1):Employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: a) Finish Department: On or about February 07 2023 and at times prior, the employer exposed employees to chemical hazards, in that a written Hazard Communication Program was not developed and implemented for employees using corrosive Wood Bleach and "Dura Seal" Quick Coat. b) Flooring Department: On or about February 07 2023 and at times prior, the employer exposed employees to health hazards, in that a Hazard Communication Program was not developed and implemented for employees using Wood Flour Cement, "Wonderbond" Catalyst and Polyvinyl Acetate Resin.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) At Flooring Production: On or about February 07, 2023 and at times prior, the employer did not develop and maintain certification of forklift operator training for the employee who operates the "Doosan" model GS2N-7 forklift (serial number FGA14-1290-02017) in the performance of assigned job duties.
