29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) Facility: On or about February 15 and at times prior, the employer exposed the employees to potential fire hazards, in that effective housekeeping practices were not developed and implemented where operation of equipment, including the Saw Stop Table Saw Model ICS73480 Serial Number 081301662, allowed the accumulation of combustible wood dust to a depth of 1/8 inch on elevated horizontal surfaces. A bulk sample collected on February 15, 2023, and tested for explosivity was determined to have a Kst value of 14.57 bar meters per second. b) Flooring Production and Finish Side: On or about February 15 2023 and at times prior, the employer exposed employees to slip hazards, in that the employer did not ensure the walking-working surfaces were free of wood dust accumulation while employees were performing cutting, sanding and staining wood flooring products.

29 CFR 1910.94(a)(4)(i): The construction, installation, inspection, and maintenance of exhaust systems shall conform to the principles and requirements set forth in American National Standard Fundamentals Governing the Design and Operation of Local Exhaust Systems, Z9.2-1960, and ANSI Z33.1-1961, which are incorporated by reference as specified in �� 1910.6. a) Dust Collection System: On or about February 15 and at times prior, the employer exposed the employees to fire hazards, in that the wood dust collection system was not properly maintained in accordance with the manufacturer's recommended guidelines.
