29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. 29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about February 14, 2023 the employer failed to provide documentation showing the employees engaged in commercial roof sheathing activities at a height above 10 feet were trained to recognize or minimize fall exposures, exposing employees to a fall.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about February 14, 2023 employees were exposed to fall hazards in excess of 10 feet while engaged in commercial roof sheathing activities. Fall protection was not being utilized while performing this activity.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
