29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): a- 37, 39, 41, 43, 45 & 47 Stanton Road, Mount Holly NJ: A company worker was exposed to fall hazards up to and in excess of 20 feet 7 inches to the ground surface below when utilizing an aluminum extension ladder whose rise to run ratio was not 4 to 1 and, as such, did not have an angle of approximately 75 degrees from the horizontal . Condition observed on or about 02/15/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. As such, these workers were exposed to fall hazards up to and in excess of 25 feet to the ground surface below. b- Back Roof of Structure - 37, 39, 41, 43, 45 & 47 Stanton Road, Mount Holly NJ: On or about 02/15/23 a worker was not protected from falling when walking and working from pitched roofs while wearing a full body harness that were not connected to an anchor with a lanyard or other approved means . As such, this worker was exposed to fall hazards up to and in excess of 25 feet to the ground surface below. c- Front Roof of Structure -37, 39, 41, 43, 45 & 47 Stanton Road, Mount Holly NJ: On or about 02/15/23 a worker was not protected from falling when wearing a full body harness whose connected lanyard's other end was NOT connected to an anchor. As such, this worker was exposed to fall hazards up to and in excess of 25 feet to the ground surface below. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLSULTY WILL RE IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
