29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about February 22, 2023, employees were performing masonry preparation work on the roof of a new residential construction house approximately 15 feet from the ground without fall protection, exposing the employees to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about February 22, 2023, an employee was using a ladder to access a roof up to approximately 15 feet high without the ladder extending at least three feet above the upper landing surface, exposing the employee to a fall hazard.
