29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  At a jobsite located at 2135 E 159th Circle, Wichita, KS 67230 employees conducting residential roofing activities were exposed to serious fall hazards of approximately 23 feet from the ceiling joists to the ground below in that fall protection was not provided.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 2135 E 159th Circle, Wichita, KS 67230 employees engaged in residential roofing activities were exposed to fall hazards when accessing an extension ladder:  1) Werner extension ladder utilized on the West side of the residential building was 23 feet from the eave of the residence to the ground Employee utilized this ladder to perform roofing activities in the edge of the roof. 2) All American extension ladder utilized on the East side of the residential building was 11 feet from the eave of the residence to the ground below. Employees accessed and exited the roof utilizing the portable ladder and it did not extend at least 3 feet above the upper landing and was not secured to the structure. 3) Werner extension ladder utilized on the South side of the residential building was 12.5 feet from the eave of the residence to the ground below. Employees accessed and exited the roof utilizing the portable ladder and it did not extend at least 3 feet above the upper landing and was not secured to the structure.
