29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access;    At the jobsite, Employees engaged in installing roofing material, were exposed to fall hazards while using a ladder that did not extend at least three feet above the roof's landing.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   At the jobsite, employees were not wearing hardhats while working below other employees that were roofing a residential house, exposing employees to struck-by hazards from falling objects.  Faustino Mendez was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.100(a)(1), which was contained in OSHA inspection number 1302411, Citation Number 1, Item number1, and was affirmed as a final order on April 2, 2018, with a final abatement date on April 11, 2018,  with respect to a workplace located at 3209 Charing Cross Road, Midlothian, Texas 76065.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite, employees performing framing activities without fall protection were not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
