On a 3:12 pitch residential roof, at a temporary worksite located at 6646 Heidi Road in Jacksonville, FL: On or about March 7, 2023, the employer exposed employees to fall hazards, in that fall protection was not utilized by the employees while performing roofing work.

29 CFR  1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by March 2, 2023:  a. Turnkey Roofing of Florida, Inc, located at 9521 Shellie Road Suite 1, in Jacksonville, Florida, 32257: On or about March 2, 2023, the employer failed for calendar year 2022, to electronically submit information from the OSHA Form 300A or equivalent by March 2, 2023. The establishment employed 164 employees and was classified in the North American Industrial Classification System as 238160 during calendar year 2022.
