29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  At 365 University Boulevard North, Jacksonville Florida, on or about March 07, 2024, the employer exposed 3 roofing employees to an 8.5-foot fall hazard from the eaves, in that the roof access ladder was not extended 36 inches above the roof surface.

29 CFR  1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  a.  At 365 University Boulevard North, Jacksonville Florida, on or about March 7, 2024, the employer exposed employees to a ladder collapse hazard in that the employees accessed the roof using the fly-section of an extension ladder that was not designed by the manufacturer for placement on the ground.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. Employees performing roofing are not protected from fall hazards by the use of a fall protection system.  Turnkey Roofing of Florida fails to assure effective protection against falls as required by the standard, as demonstrated at 365 University Boulevard North, Jacksonville Florida, on or about March 07, 2024, when the employer exposed 3 roofing employees to an 8.5-foot fall hazard while removing old roofing system materials, in that the employees were not protected by a fall protection system while performing residential re-roofing.  The Turnkey Roofing of Florida, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1654844, citation number 01, item number 001 and was affirmed as a final order on 4/25/2023, with respect to a workplace located at 6646 Heidi Road, Jacksonville, Florida 32277.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  a. At 365 University Boulevard North, Jacksonville, Florida, on or about March 07, 2024, the employer exposed employees to fall hazards, in that the employees were not trained to recognize hazards related to the extension ladders used for roof access and soffit work.
